Why medical groups offer Alice

A 10 to 100 clinic medical group has heavy frontline headcount (MAs, CNAs, front desk staff, billing staff, schedulers). Its corporate HR team is already running healthcare benefits, retirement, and payroll. Adding Alice does not add per-clinic labor.

The rollout flows through the existing payroll connection. Employees don't pre-fund. The employer keeps a reserve amount in its Alice plan account; it belongs to the employer and is not a fee. Traditional pretax benefit providers were built for office workers: you pay up front and manage enrollment and payroll yourself.

What medical group employees get

Every eligible W-2 employee at every clinic gets Alice Card, a Visa commercial credit card. The card works at transit fare gates, commuter rail vending, bus fareboxes, parking garages, parking lots, parking meters, and parking apps.

The 2026 IRS limits are $340 per month for transit and $340 per month for qualified parking (Rev. Proc. 2025-32). Uber and Lyft rides are not eligible, except for qualifying vanpools.

How multi-clinic payroll connections work

Alice connects to 30+ payroll providers, including connections to ADP, UKG, Paylocity, Paycom, Paychex, Toast, and others. Medical groups commonly run ADP, UKG, or Paychex across every clinic, often with each clinic as a separate EIN.

Alice maps each employee to the correct EIN inside the payroll feed. Deductions and tax savings post to the legal entity that employed them in that pay period.

Setup speed

Setup starts with one call. The corporate HR call kicks off the connection. Rollout to employees across every clinic typically completes within two weeks, depending on how quickly payroll credentials are shared.

How much medical groups actually save

Employers save FICA on every pretax dollar an employee runs, compounding across the headcount of the entire group. Employees save about 20% on transit and qualified parking. Actual savings depend on each employee's tax situation.

FAQs for medical groups

How are physician partners treated?

Owners and certain partners are excluded under the federal commuter benefit rule (§132(f)). W-2 employed physicians and W-2 staff are eligible.

What about new clinic acquisitions?

Employees at the acquired clinic roll under the existing Alice contract through the same payroll connection.

Do residents and fellows qualify?

If they are W-2 employees of the group, yes. Stipend-paid trainees outside W-2 employment generally do not qualify.
